Our 245D program is a Minnesota licensed Home and Community Based Program that provides 24 hour Emergency Assistant, Personal Support , Homemaking Services, Night Supervision, Respite care, Adult Companionship services, Individual Community Living Support,  Individualized Home Support and  Independent Living Skills.

Staff

 At Nobel Health, we understand that not one service plan can provide all the help one may need. That's why we offer personal care services that are customized to each client's needs and preferences whether it is 24 hour night supervision, companionship, meal preparation, hygiene, cleaning, and other homemaking service

OUR SERVICES


  • 24-Hour Emergency Assistance: Immediate support available at any time, day or night, for unexpected situations or crises.
  • Personal Support: Assistance with daily activities like grooming, meal preparation, or mobility, designed to help individuals live as independently as possible.
  • Homemaking Services: Help with household tasks such as cleaning, laundry, shopping, and meal planning, which allows individuals to maintain a safe and clean living environment.
  • Night Supervision: Monitoring and support during the night to ensure the individual's safety and well-being, addressing any concerns that may arise during sleeping hours.
  • Respite Care: Temporary care that provides relief for primary caregivers, allowing them time to rest or manage personal matters.
  • Adult Companionship Services: Social support and interaction to prevent isolation, fostering emotional well-being through activities like conversation, shared hobbies, or outings.
  • Individual Community Living Support (ICLS): Customized assistance that helps individuals with skill-building and support to remain active and engaged in their community.
  • Individualized Home Support (IHS): Tailored services that focus on achieving personal goals, promoting self-sufficiency, and enhancing independence within the home.
  • Independent Living Skills (ILS): Training and guidance in skills like budgeting, cooking, or personal care, aimed at helping individuals manage their daily lives with greater independence.

These services are designed to improve quality of life, promote independence, and provide the necessary support for adults with special needs in a variety of areas.
