STEPS OF THE REFERRAL PROCESS:
The case manager can refer to our agency either by phone or through the online referral form on our website
Upon referral, we schedule an intake meeting within 10 working days. This is a pivotal step in getting started, offering options for in-person, phone, or video conferencing meetings.
The intake meeting includes signing admission paperwork, reviewing our policies, and providing an opportunity for clients to ask questions about starting services with us.
After the intake meeting, we create a personalized support plan based on each client's unique needs and goals. This plan is then shared with the client and their care team, playing a crucial role in beginning care with Nobel Health.
Within three weeks of the intake meeting, we’ll match the client with a caregiver who fits their needs and goals. We make sure the client is comfortable with their caregiver, who will have the right skills and experience to provide quality care.
We will regularly conduct quality assurance reviews to ensure the client is receiving high-quality care and that their support plan is being followed effectively. We will collaborate with the client and their care team to identify any areas for improvement and make necessary adjustments. This ongoing process is a key part of ensuring continuous, effective care.
